Health Care Directive / Advanced Planning
Individuals should have a current Health Care Directive (HCD). A current HCD is a gift to your loved ones and helps you get the care you would choose if you were able.

Making my Wishes Clear offers statements one can review to help clarify their health care wishes and ensure their health care directive is clear and usable.
The Provider Orders for Life Sustaining Treatment (POLST) form is a portable medical order that can give patients with advanced serious illness the option to exercise increased control over the treatment they do and do not want to receive at the end of their life. The POLST form is NOT the same as a Health Care Directive, but it may be an important component of advanced planning in certain circumstances. Unlike a healthcare directive, a POLST form must be signed by a liscensed provider to be valid.
