The primary function of this position is to assess and provide support to older adults and their family caregivers to help them remain in the home of their choice.
This position is accountable to the Executive Director (ED) and works with other staff members and the board of directors of Helping Hands Outreach to accomplish the organization’s mission. The position hours can be 36 or 40 hours per week at $21 per hour.
Responsibilities include –
- Complete initial and yearly assessments of clients to determine what supports are needed for them to remain in their homes. This includes the Rapid Screen® and NAPIS Registration form.
- Provide information and referrals to clients and their families. Assist with applications for eligible programs.
- Promote caregiver support and health & wellness events and activities through the newsletter, Helping Hands events, and personal contact.
- Develop and maintain resources and knowledge on health, wellness, and caregiving, including attending training and conferences when necessary.
- Willing to be trained and certified as a Caregiver Consultant to provide one-on-one consultations for caregivers.
- Maintain program and service data in the database and other information required by funders.
- Provide respite to caregivers when necessary.
- Maintain and enhance professional skills and network with other persons/agencies who provide senior services, caregiver support, and health and wellness information and training.
- Attend staff and board meetings as requested.
- Assist other staff with community programming.
- Accomplish other tasks that may be assigned by the ED.
- Work collaboratively and creatively to achieve the mission of the organization.
Qualifications:
- Bachelor’s Degree in a related field
- Experience with older adults and human services
- Detail-oriented with strong computer skills
- Familiarity with Microsoft Office
- Caregiver or Caregiver Support experience preferred
